Justification Outline for Awards Nomination

Under the appropriate categories indicate why the person/group is deserving of an award. Provide specifics on how the person/group exceeded expectations as identified in the current position description of their work, advanced the mission of the agency beyond expectations. Do not limit your statement; for example, they used unusual creativity – give specifics indicating what the person/group did that was unusual or creative. The awards committee ranks nominations based on the information provided in this attachment.

The justification for all categories should not exceed 2 pages. The justification can be completed on this form. Not all categories need to be completed. Performance award nominations require a minimum of 3 categories to be addressed. Fill out only those applicable to this award.

  2. How has the employee’s work resulted in the timely completion of a project?

  3. How has productivity increased as a result of the employee’s work?

  4. How did the employee overcome adverse conditions or unusual circumstances?

  6. How did the employee save time, money, materials or increase funding for the Agency or the State?

  7. How did the employee’s work result in increased program effectiveness or acceptance?

  8. How did the employee’s work result in increased cooperation?

  9. How did the employee’s work result in technology advancement?
